Easy-read HAP and PCP documents

Every plan can be downloaded two ways: the standard black-and-white corporate document for professionals and printing, and an illustrated easy-read version made for the person, their family and advocates. Same content, two audiences.

  • Standard and individualised versions
  • Download one plan or all of them
  • Illustrated with the person's own photos where you choose

Documents with real folders and a bin

Hospital passports, consent, DoLS, letters, PBS plans, nutrition charts. Blue folder tiles six levels deep, drag files from the desktop, take a photo on the phone, one tap creates the standard folder set. Nothing is deleted, only binned and restorable.

  • Standard folder set in one tap
  • Every upload checked by its bytes, never its name
  • Search across every folder

Important people, visits and the profile PDF

Family, advocates, GP, social worker and every professional who visits. A visit by a family member writes itself onto the daily log. The profile PDF lets you tick the sections you need and produces a corporate A to Z document in seconds.

  • Visit reports with totals and trends
  • Per-user tab order, drag to reorder
  • Goals with milestones and a photo memory book

Questions

Things managers ask.

Can families read it?

The easy-read HAP and PCP documents are made for exactly that. Families and advocates get an illustrated version in plain language.

Can we bring our current care plans across?

Yes. Setup includes building current plans with you, and importing what your old system will export.

Who can see what?

Permissions are per person, per module, read or write. Staff assigned to a home see that home only.
